Our Deaf and Hard of Hearing team provides sign language interpreting services as requested by the school districts in our service area.

Students who would like to request sign language interpreting services for school-sponsored activities occurring outside of the regular school day must request those services using the Sign Language Interpreter Form linked below.

Sign Language Interpreter Request Form

Directions for Completing the Form

  • Complete the form by providing all requested information. Please note that missing or incorrect information will increase the processing time of your request. If the interpreter request is for an recurring event (e.g., athletics, theater productions, overnight weekend activity) it is helpful to provide a copy of the event schedule.
  • Students under 18 years old must have a parent sign the request. Students who are 18 years and older may sign the request for themselves.
  • Fax or mail the completed form and activity schedule (if available) to Columbia Regional Program DHH services at:
    • Email: jgoshman@pps.net
    • Mail: 833 NE 74th Ave Portland OR, 97213
    • Fax: 503-916-5576

How long will it take to process the request?

Columbia Regional Inclusive Services must contact the student's school district for approval before arranging for any extra curricular interpreting services, so please allow approximately 5-7 school days to process your request.

If you have any questions, please email Jennifer Goshman.
